Adding additional collaborators emails to placement-announce list
You must update your "Personal Preferences" before starting a new questionnaire. One individual cannot create a theme camp. Be sure to list the emails (not names) of your camp collaborators in the "Addition Emails" field, one address per line. After submitting your questionnaire, any changes need to be updated by you in your personal preferences and sent to placement@burningman.com.

You may save and return to your questionnaire as often as you need by using the SAVE button at the bottom of the questionnaire. However, you must SUBMIT before the deadline. You will receive an email confirming the receipt of your submitted questionnaire.

The only way to submit your answers to the questionnaire is through the website. Please DO NOT mail or email your questionnaire answers. This will not give us all the information we need. Many participants have found it useful to preview a copy of this questionnaire. To that end, we have provided a PDF version of the Placement Questionnaire for this purpose only.
